Memorial Quilt Program

CorneaGen’s memorial quilt program is an opportunity to honor and remember cornea donors who have made the Gift of Sight possible for those suffering in darkness. Each unique square is created and submitted by donor families to memorialize their loved one and displayed as a visual reminder to those who have selflessly given of themselves so others can see.

The quilts are used as educational tools to promote awareness of cornea and eye donation at hospitals, health fairs, and community events to spread the message of cornea and eye donation and transplantation.

Donor families are invited to contribute a patch to the quilt. Each patch tells a unique story, and they are always accepted. Your design should be a unique expression of your loved one’s life and legacy.

Quilt Patch Instructions

Download the template here and mail in your quilt patch with your completed Donor Family Release form.

Size

Your quilt patch must be an 8” x 8” square, allowing for a 1/2” seam on all four sides (design should be within 7” x 7”). It’s very important that your patch meets the size guidelines (see sample patch outline or it will be very difficult to include in the quilt and may be returned.

Fabric and Sewing

Please sew everything securely and avoid using glue!
If you are including a photograph on the patch, have it transferred directly onto the fabric. (Iron-on fabric transfers and/or printable fabric sheets can be purchased at craft and retail or office supply stores).
You may use any fabric color or type, including sentimental materials such as your loved one’s baby blanket, high school jacket, tie, dress or t-shirt.
You may use crayons, paint, or permanent markers to design the patch.
DO NOT GLUE ITEMS TO THE PATCH. They tend to fall off over time.
Feel free to include your loved one’s name and dates of birth and death, poems or quotes.
Do not “finish” the patch with quilting or backing
